Insurance
Insurance plans differ from one another in regards to benefits and coverage policies and so it is important that you provide us with information about your insurance plan. When you visit us please bring with you your updated insurance card along with a photo ID.
To make sure you get the coverage you need it is important that you understand what rules go along with your insurance plan and what portion of the bill you will be responsible for. To find out what your insurance plan covers and what your financial obligations are, please contact your healthcare insurance company’s customer service department.
Your insurance company may require that you receive authorization from them prior to your hospital visit. Please make sure you obtain any authorization needed or you could be responsible for paying your bill in full. For any questions about this please contact your healthcare insurance provider.
Financial Assistance
If you need help paying for hospital services, you may request a financial assistance application by contacting the Business Office at 756-3530. In addition to the form, a list of required documents will be sent to you. Some of the documents needed include copies of tax returns and pay stubs. After the application is filled out and signed it will be reviewed by the hospital and then you will be notified of your approval or denial. You may also request to set-up a monthly payment plan to suit your budget.
Billing Facts
During your hospital visit you may receive a variety of different services and treatments. Each service provided to you during your stay may be on a separate bill. Visits by your physician and any consulting physicians will be billed seperately by their offices. The actual hospital bill may include charges for your hospital room, medication, lab tests, X-rays and any additional items. For outpatient care, your bill may include physician fees for diagnostic testing, outpatient procedures, and charges related to tests and equipment used at the hospital.
If you have insurance, we will send your insurance company a copy of the bill. After they receive it, they will send back information about what, if any, amount you will owe from that bill. If you owe on your balance, it is due 30 days after the bill has been sent. If your insurance company does not pay their bill on time, 90 days after they receive it, you will be mailed a letter stating that your insurance company has not paid us. If this happens, please call your insurance company. You, as the patient, are responsible for making sure your bill is paid.
Medicare and Medicaid Tips
Medicare and Medicaid does not pay for every medical service. Medicare coverage rules are based on your medical condition as well as the number of times a particular service was provided for you in the past. Please make sure a particular test or service is covered before it is provided for you.
