Our goal is to provide disabled, chronically ill, and elderly persons with the care necessary to safely remain at home. We understand that each patient has specific healthcare needs. The Long Term Home Health Care (LTHHC) program is tailored to the individual. Our program combines medicial rehabiliation, social, and environmental treatment.

How to make the LTHHC program part of your life
Patients may enter the program upon discharge from a hospital or nursing home, or they may apply directly to the program. Family members may also submit an application for a patient. We will then schedule a visit to determine if the program is appropriate for the patient. The next step involves development of an individualized LTHHC plan upon a physician's recommendation.
Requirements for using the program
- Patients must be eligible for nursing home care and have approval from their physician
- Medicaid participants also need the approval of the NYS Department of Social Services
- A family member or responsible adult must be willing and able to provide necessary support when required
- In most cases, the patient's home care costs must not exceed 75% of the cost of institutional care in Cortland County
All fees for services are based on the cost necessary to provide that service. Usually these fees are covered by: Medicare, Medicaid, Private Insurance, or Private Pay.
