Clinical Documentation Integrity Specialist

Responsible for facilitating the improvement in the overall quality and completeness of medical record documentation. The CDI Specialist will obtain appropriate clinical documentation through daily interaction with physicians and other members of the patient’s Interdisciplinary Care Team which will support appropriate severity of illness, expected risk of mortality and the complexity of care provided to patients. Explicitly, the CDI Specialist will seek to clarify unclear or conflicting medical record documentation by communicating with practitioners face-to-face or via the clinical documentation query process in order to obtain concise clinical documentation for coding all diagnoses and treatments rendered to hospitalized patients. The CDI Specialist will provide ongoing education to clinical staff regarding the importance of complete and accurate documentation to support accuracy in coding and hospital reimbursement; along with quality-of-care initiatives, present on admission indicators and outcomes assessment. Finalization of coding and DRG assignment will be performed on each case reviewed in conjunction with appointed members of the H.I.M. Coding Staff.
Education/Qualifications:
Graduate of accredited registered professional nursing program with an AAS or Diploma in Nursing
Experience:
Three years recent acute care clinical nursing experience required. Three years acute care Utilization Review/Case Management experience a plus. Prior experience in Clinical Documentation Improvement preferred.
